Kaiser Permanante – an Integrated Delivery Network (IDN)
Kaiser Permanente is one of the nation’s largest not-for-profit health plans and concurrently serves as an integrated delivery network serving more than 9 million members, with headquarters in Oakland, California. Kaiser engaged PluralSoft and the use of CareQuotient™ to assess the availability of, and characterize, all clinical data within Kaiser’s clinical application systems (includingEpic). This effort was conducted to assess the completeness of NQF endorsed eMeasure specifications and to develop a blue-print for an architecture required for a robust clinical quality measure development-computation-monitoring-publishing application. Impressed by our work and seeing the impact of the work on the entire organization, Kaiser Permanente co-authored a research paper which was published in October 2014 edition of Journal of American Medical Informatics Association. CCMCN – a Health Center Controlled Network
The Colorado Community Managed Care Network (CCMCN) is a Health Center Controlled Network serving 13 Federally Qualified Health Centers in Colorado. CCMCN needed to help its member organizations—local community health centers that form the state’s healthcare safety net—meet government grant reporting mandates and analyze the quality of care across their patient populations which are driven by governing federal and state initiatives such as the Colorado Medicaid ACO/PCMH initiative currently underway.
CCMCN chose PluralSoft as the health information technology partner, among a competitive bid of 35 vendors, to leverage PluralSoft Clinical Analytics solution as well as our technology and domain expertise to create the CACHIE Analytics platform. PluralSoft integrated data directly from multiple instances of NextGen EHR, GE Centricity and eClinicalWorks besides integrating Medicaid Claims data from Colorado Medicaid Management Information System across 4 Federally Qualified Health Centers, a large IPA affiliated with Centura Health in North Denver/Boulder area of Colorado, and an ACO organization in Southern Colorado consisting of 4 Federally Qualified Health Centers, a Behavioral Health Center, two Private Physician Practices and a Hospital.
